Cruxpoint Health Breakthrough is a Two-Phase Program

Phase I: Diagnostics & Assessments
Phase II: Customized 24-Month Health Breakthrough Program

Phase I: Diagnostics & Assessments
Phase I is generally about two months, costs $3,200 and includes the following:

All consultations with practitioners and coaches
Cruxpoint Intake Forms: Assessments and review
Review of past medical records, documents, and treatment plans
Customized Comprehensive Labs (Vibrant America)
Review of comprehensive labs
Review of Coronary Artery Calcium Scan (CAC Heart Scan) if indicated
Review of client’s medications and supplements
AI-assisted diagnostics (if requested and consented by client)
Goal-setting: Clear goals and outcomes for Phase II

NOT included in the Phase I fee:

Outside labs and scans other than Cruxpoint’s Comprehensive Labs
Outside medical providers and services
Mental health services
Medications and supplements
The customized treatment plan (This is part of Phase II)

Phase II: 24-Month Health Breakthrough Program After completing Phase I, Phase II begins — A 24-month personalized treatment program for those who choose to go forward. The cost is $295.00 per month for the 24-month period. What’s NOT Included in the Phase II Program Fees.

Labs, tests, scans, or other diagnostics and imaging (except for Cruxpoint’s initial Comprehensive Labs, which are included)
Visits to outside medical professionals or specialists
Medications, supplements, over-the-counter products, or vaccines

A La Carte Items
The following optional offerings are available through Cruxpoint and can be selected
at any time during Phase I or Phase II. These are charged as indicated:

Mental Health Services with PhD Psychologist: $295/hour
Review of Cardiex/ConneQt Vascular Assessment: $295/hour
Review of InBody Body Composition: $295/hour
ABSI Mortality Risk Assessment & Review: $295/hour
Goldenson Lifespan Assessment & Review: $295/hour
Circle of Life Assessment & Review: $295/hour
